Does that mean a redesign of the Max door/hatch? - Joe King of Geelong.

And not before time. Unfortunately it has a sense of shuffling the deck chairs on the Titanic kind of feel to it and doesn’t go far enough to address the problems at Boeing. With pressure coming from Emirates and a couple of other heavy hitters I’m sure there will be some more movement within the organisation shortly and I can’t see them being happy with the Fleming appointment

What is that? Weird looking failure.

Wing slat. Probably lucky it was the trailing edge of the slat otherwise it may have been far problematic

No, I meant what sort of failure? Is that composite structure? Just the type of failure looks weird

Edit: maybe that’s just the paint that I’m looking at?

Composite delamination.
It’s a 30 yo aircraft.
Speculating here, the slat suffered water intrusion. An overnight freeze cycle will bust apart a composite after a few goes if water is getting in.

A TUI Boeing 787-8 Dreamliner was damaged after being involved in an incident at Birmingham Airport. The front door of the plane got stuck on the stairs outside when it was opened. No injuries were reported.

It looks eerily similar to the damage one of our freighters received from a bird strike last week.
Looking at the history of this aircraft it seems back in November it was also diverted back to SFO due to tech issues and was out of action for a week.

She did a great job early on but under her watch Virgin has had a real bad couple of months and not ideal when you trying to launch a IPO. Bain don’t mess around no matter how loyal you have been to them

Pakistan has released the final report on the PIA A320 crash at Karachi airport 2020.

IIRC this is the one where the A320 landed gear up, destroyed it’s engines, attempted a go around and crashed.

It’s not pretty reading.

  1. totally unstabilized approach to the runway

  2. ignored sterile cockpit rules

  3. ignored ATC direction to go around (ATC could see the plane was way off glideslope)

  4. ignored gear up notifications.

  5. tried to go around once it was on the deck.

The list is long.

Primary Causes

- Aircraft made Gears Up landing where both Engines’ nacelle made contact with R/W. Both Engines were damaged causing loss of Engine Oil and lubrication which resulted in failure of both Engines during Go-Around.

  • Non-adherence to SOPs and disregard of ATC instructions during the event flight.

  • Lack of communication between the ATC and the flight crew regarding Gears Up landing particularly once aircraft was on the R/W.

Contributing Causes

  • Ineffective implementation of FDA programme.

  • FDA regulatory oversight programme was ineffective in producing sufficient and timely improvement.

  • Lack of clear and precise regulations to restrict flying while fasting.

  • Inadequate level of CRM application during the event flight.

PIA are one of the worst in the world so no surprise with the findings.
They were actually banned from flying to Europe and the UK only a few years ago as over a quarter of the pilots had fake licenses and some had no formal qualifications at all.
They have applied to fly to Australia but I won’t be jumping on :rofl:

Air Crash Investigation episode on this would go for hours given all the failings.

It’s totally unbelievable they tried to land that plane, despite ATC waving them off, and even more unbelievable they tried to do it forgetting to put the gear down. It’s one of the worst I’ve read.

Pilot probably wasn’t even licensed or trained, I mean some of those factors are beyond comprehension
