'Ghost flight' horror crash blamed on pilots

Human error by two pilots led directly to a packed passenger plane turning into a frozen "flying tomb" before it crashed - killing all 121 on board, the official report has concluded.

It blames the pilots for failing to spot the warning signs that led to the tragedy in which the Boeing 737-300 airline was hit by a catastrophic loss of cabin pressure and oxygen supply.

This turned the plane into a "flying tomb" at 34,000 feet as passengers were frozen to their seats by temperatures of minus 50c.

A steward fought desperately to save the stricken plane after he found the pilot and co-pilot unconscious.

But after he too was overcome, the Cyprus-based Helios Airways plane ploughed on for nearly two hours on auto-pilot before running out of fuel and crashing into a mountain 25 miles north of Athens on August 14 last year.

The official report says technicians in Cyprus who were checking the decompression system - following problems on an earlier flight - forgot to switch on its automatic activation.

But once airborne, this omission was compounded when pilots forgot to check whether the system was switched on automatic or manual.

As a result, the higher the plane flew the less oxygen was in the cabin.

This caused everyone aboard, except for the steward Andreas Prodromou, 25, to fall unconscious.

The remains of Mr Prodromou, who had trained as a pilot but been unable to secure a job, were found in the cockpit.

His girlfriend a stewardess, was also on board.

His heroics were witnessed by pilots of two fighter jets sent up when Flight ZU 522 lost radio contact.

It crashed at Grammatiko, north of the Greek capital.

All 115 passengers - all Greek Cypriots and Greeks - and six crew were killed on the flight from Cyprus to Prague which is the deadliest air disaster in the history of Cyprus and Greece.

The report concluded that the two pilots failed to competently operate controls regulating cabin pressure and misinterpreted a subsequent warning signs which led to the crash.

Specifically, the crew failed to recognise that the plane's pressurization switch was in the "manual position" before takeoff and not set to automatic - which would have allowed the cabin to pressurise by itself.

After take-off, the plane failed to pressurise and the two pilots did not recognise "the warnings and the reasons for the activation of the warnings," including one showing that the oxygen masks dropped.

That failure led to the "incapacitation of the flight crew due to hypoxia", or lack of oxygen, and resulted in the plane being flown on autopilot for two hours until it ran out of fuel and crashed.

The captain contacted Helios's operations centre after take-off.

As the aircraft climbed though 16,000 feet (4,879 meters), he reported warnings going off in the cabin, said the report.

"Several communications between the captain and the Operations Centre took place in the next eight minutes concerning the above problems and ended as the aircraft climbed through 28,900 feet (8,810 meters)."

"Thereafter, there was no response to radio calls to the aircraft," the report said.

During the climb, "the passenger oxygen masks deployed."

The report did not say if any of the passengers had managed to put on the masks.

But it confirmed that at least one man without a mask was seen alive in the cockpit by the pilot of a Greek F-16 fighter jet scrambled to intercept the airliner who "tried to attract his attention without success."

The plane issued two "Mayday" signals when the first engine cut out and crashed 13 minutes later after the second engine cut out, it said

Earlier this year, Mr Tsolakis said the flight attendant had wrestled with controls for at least 10 minutes before the crash, in a failed effort to regain control of the airliner.

Helios Airways Flight 522 had been travelling from Larnaca, Cyprus, to Prague, Czech Republic, with a scheduled stop in Athens.

Maintenance officials in Cyprus also took a share of the blame along with Cypriot civil aviation authorities.

The airline was also slated for "deficiencies in the organization, quality management, and safety culture."

The report also cited the aircraft's US manufacturer, Boeing, for "ineffectiveness of measures taken in response to previous pressurization incidents in the particular type of aircraft."

And official aviation authorities were also criticised for "inadequate execution of its safety oversight responsibilities" over time.

Helios Airways, a budget Cypriot airline, was replaced earlier this year by a new carrier called ajet.

The report was prepared by Akrivos Tsolakis, head of Greece's National Aviation Safety Board

and delivered to Greece's transport minister.

The revelation comes amid a desperate search for traces of the plane with 239 people on board, which lost contact with air traffic control at around 1730 GMT Friday, about an hour after take-off from Kuala Lumpur en route to Beijing.

"We are issuing this AD (Airworthiness Directive) to detect and correct cracking and corrosion in the fuselage skin, which could lead to rapid decompression and loss of structural integrity of the airplane," the Federal Aviation Administration said.

During a sudden drop in cabin pressure, the crew and passengers can become unconscious, leaving no one at the controls of the affected aircraft.

In 1999, a Learjet carrying golfer Payne Stewart crashed into a field in the US state of South Dakota after flying uncontrolled for several hours after those on board apparently became unconscious due to a lack of oxygen brought on by a loss of cabin pressure.

Malaysia's air force chief has raised the possibility that MH370 inexplicably turned back and was quoted as saying the jet had been tracked hundreds of miles from its intended flight path.

The FAA circulated a draft of the directive warning of the cracking and corrosion problem on September 26, 2013.

A final directive was issued March 5, three days before the Malaysia Airlines jet disappeared. The FAA directive is to take effect April 9.

The FAA said it was "prompted by a report of cracking in the fuselage skin underneath the satellite communication (SATCOM) antenna adapter."

"This AD requires repetitive inspections of the visible fuselage skin and doubler if installed, for cracking, corrosion, and any indication of contact of a certain fastener to a bonding jumper, and repair if necessary," it added.

The FAA said an estimated 120 US registered aircraft are affected by the directive.

A supporting document accompanying the directive indicates that one unidentified operator reported a "16-inch crack" (40.6- centimeter) of the fuselage skin in an aircraft that was 14 years old.

Boeing performed a "metallurgical fracture analysis" of the affected section of the fuselage skin, according to the document.

As the hunt for the missing plane dragged into its fifth day, Malaysian authorities said they were expanding their search zone off Vietnam's South China Sea coast to the Andaman Sea north of Indonesia, hundreds of kilometers (miles) away.

This particular ep was on again on monday night or tuesday.

 

Somewhat freaky that a plane could not only fly the majority of the journey by itself, but also fly in a holding pattern around the airport.

 

Also worryin was the fact that the masks can drop down in the cabin, and not necessarily inthe cockpit which you'd think would be mandatory on any occassion that the cabins masks fall down.

 

The show also raised the questions what the cabin crew guy was doing, as they had 4 air bottles that could last for an hour were found used, in the crash site.

 

Not that it'd have helped those on board, as they were going to be vegetables if they had survived, but still.

Good point.
He was the sole survivor that either braindamaged or killed some hundreds of people, untrained (or undertrained), all the qualified people had screwed up leading to the situation... CLEARLY should have done better with the opportunity presented him.

The engines transmitting data for a further four hours after the transponder stopped sending was probably known from very early on. 

 

It's only just become public now for some reason.

With all the modern technology at our disposal I am shocked that almost a week later there is no sign of the plane. The transponder being off is a big worry. Why? By who?

As you say with all the technology nothing.

Then you think about the technology on the plane via passengers, and surely one of them could have got some info out if they were in trouble.

If something happened suddenly and no one had time, you'd think they'd have found at least something by now as to a crash site.

 

The longer it goes on, the more questions will arise.

Seems the US electronic ELINT birds picked up the SATCOM “pinger” for several hours after loss of contact.

That’s why the Malaysians can honestly say they didn’t actually get the messages.

Well considering their ability to find Bin Laden & WMDs, I would encourage every other country to send naval vessels to the Pacific Ocean instead.
